The Clinical Nurse Liaison will work closely with hospital leadership to identify potential patients for admission to Vibra Hospital, and will perform required clinical assessments on patients requiring skilled nursing, long-term acute or specialty services. Assessments will be completed in a timely fashion, preferably on site which will include chart review, patient interview/assessment, communication with case manager, physician, nursing and other involved interdisciplinary team members, as well as family as needed. The Liaison will proactively identify and develop potential referral sources and maintain contacts with established referral sources in order to achieve targeted admission goals. Referral source development and maintenance is a critical aspect to the position.

Skills / Requirements

Qualifications for Clinical Nurse Liaisons:
Marketing/sales experience in Pharmaceutical, DME, Home Health or other healthcare/medical setting is desired.
Bachelor‚s degree in related field preferred.
RN degree from an accredited school of nursing is required.
ICU/CCU experience required.
Relationship building skills, negotiating skills strongly preferred.
Unrestricted OR drivers license.
Current immunizations (MMR, DPT, Varicella, Hep B and TB screening) are required.
